The FHSAA has recently updated their EL2 Physical Forms. All current, up to date physcials on file are still good for a calendar year, however any new physcials moving forward must be done on the new forms.
The new EL2 forms are 4 pages. The 5th page is only there if you are referred to a specialist.
Page 4 of the EL2 form is the only form that says "Submit this medical eligibility form to the school".
You also will need to complete the Consent & Release from Liability Certificate (EL3).
