- Preparticipation Physical Evaluation- EL2
- View NFHS Learn Videos at nfhslearn.com/courses

Student Athlete must take these free courses. The completion certificates must have the student athlete's name.
Once completed please upload the certificates to Athletic Clearance athleticclearance.fhsaahome.org
- Concussion in Sports Course
- Sudden Cardiac Arrest Course
- Heat Illness Prevention Course

Things to know:
1. You must update your information every school year. Even if nothing has changed, you must
give consent for your child to participate each year.
2. You must choose every sport your child is interested in playing.
3. All physicals must be on the FHSAA paperwork. 

4.  All student athletes must have insurance.  If you need to purchase insurance for your student, use this link: https://schoolinsuranceofflorida.com/
5. Must have the student and parent signature (signature page)
6. Students must bring proof of clearance to their coach before they can participate in conditioning
and/or tryouts. (Example: Confirmation email or My Clearance page)
