ONLINE ATHLETIC CLEARANCE
If your student intends to participate in any school related athletics you must complete and athletic clearance. Students will need a sports physical and to watch required videos and then upload the certifications upon completion.

- Concussion in Sports Course
- Sudden Cardiac Arrest Course
- Heat Illness Prevention Course
Everything you need will be found in this link: https://athleticclearance.fhsaahome.org/ Once you create an account, all completed physical paperwork and certificates must be uploaded to this site.
Preparticipation Physical Evaluation-EL2
Things to know:
1. Once you create an account in the Athletic Clearance Site, you will continue to use that account year after year to renew your student's information and give consent for your child to participate.
2. Choose every sport your child is interested in playing, even if you are not sure they will.
3. All physicals must be on the FHSAA paperwork.
