Notice to parents about school health services: Without a signed Health Services Consent form, school clinic staff cannot assist students. Parents are urged to return the form promptly or complete it online via the Skyward portal. HB 1557 mandates that schools provide a list of available health services and allows parents to decline consent for specific services.

CARE PLANS:

Cardiac Care Plans must be completed and signed by your students Cardiologist. All other Care Plans may be completed by parent/guardian.  Please provide as much detailed information as possible to better help school staff to care for your child at school.
