2023-2024 Project Connect Meetings

The mission of the Hernando County Project Connect Transition Interagency Council is to increase agency and parent/family involvement at the school level. It is our intent to assist students with disabilities in transitioning to adult life by empowering them with the knowledge and tools to become participating members in the Hernando County community.

The cooperating agencies, service providers, school districts, family members, students, and associated members of the community are committed to collaboration in the development and implementation of post-secondary training and educational options for students with disabilities. Through this interagency council, we acknowledge an awareness of the need for the provision of a full continuum of services and further, commit to the creation of a set of outcome-oriented activities for each student that will facilitate a smooth transition from high school to appropriate gainful employment, continuing education, and optimum participation in the community. The agencies and entities involved are committed to helping coordinate services for individuals with disabilities and their support networks by providing technical assistance to the local school system and rehabilitative personnel.
